Office Policies

We have your convenience in mind. With the NEW online patient registration portal you can fill out your patient registration any time of day. Registering online will save time when you arrive for your appointment. This will ensure that you are seen as quickly as possible when you arrive for your appointment. Registration only takes about 10 minutes. Before you start the registration process, have your medical and vision insurance cards at hand. Fill out the information as accurately and completely as possible. You must fill out all required fields prior to your schedule appointment for the registration process to be complete to avoid being cancel and we must receive a copy of your ID and insurance copy along with all your form prior to your appointment within 48 hours to avoid being cancel.

Appointments & Cancellations

With the exception of serious emergencies, it is expected that you keep all of your appointments. If you need to reschedule an appointment we require 24 hours notice. In such a case, please call our office and arrange for a make-up appointment with one of our patient care coordinators.

1. Cancellation/No show Policy for Appointment

We understand that there are times when you miss an appointment due to emergencies or obligations for work or family. However, when you do not call to cancel an appointment, you may be preventing another patient from getting much needed treatment. Conversely, the situation may arise where another patient fails to cancel and we are unable to schedule you for a visit, due to a seemingly "full" appointment book.
Patients who fail to show for scheduled appointments and did not notify the office within 24 hours of their appointment time will be charged a no-show fee of $50.00. In the event of an actual emergency and prior notice could not be given, consideration will be given, and a one-time exception may be granted.
2. How to Cancel Your Appointment
If it is necessary to cancel your scheduled appointment, we require that you call 24 hours in advance. Appointments are in high demand, and your early cancelation will give another person the possibility to have access to timely mental health care.
To cancel an appointment, please call our office 9:00 a.m. through 4:30 p.m. at (702) 664-9614 and speak with our schedule coordinator or you can text at this number as well.  You can also cancel by e-mail: scheduling@arkmentalhealthwellness.com 
3. Scheduled Appointments
We understand that delays can happen, however, we must try to keep the other patients on time. If you are running late, please notify the office. 
If a patient is 15 minutes past their appointment time, we will re-schedule your appointment. 
These fees are not covered by insurance and if therefore the sole responsibility of the patient.
The following are charges for services in the office:
Rescheduling/cancellation with less than 24 hours: $50.00
No show first offense: $100.00
No show second offense: $150.00
Third show: will be discharge from care

Emergencies

If the situation is potentially life-threatening, get immediate emergency assistance by calling 911, available 24 hours a day.

Payment

My office accepts the following methods of payment for co-payments, deductibles and services that are not covered by insurance:

  • Visa
  • MasterCard
  • American Express
  • Discover
  • Personal Check
  • Credit

We make every effort to decrease the cost of your medical care. Therefore, we request co-payment and deductibles at the time of service. We accept MasterCard, Visa, and Discover for your convenience. If we are a participating provider of your insurance company, we will bill them. However, payment is the patient's responsibility. We will help in any way we can to assist you in handling claims.


We also offer cash pay rate. Please call our office to discuss your situation and more information.
